Job Summary:

We seek a highly skilled and experienced Registered Nurse with specialities in Medical/Surgical, ICU, ER, Psychiatric, Labor & Delivery, PACU, Operating Room. The Registered Nurse will be responsible for providing skilled nursing care, case management, and hospice care to individuals facing advancing age, frailty, and serious illness. Daily tasks include patient assessments, medication administration, treatment planning, and collaborating with the interdisciplinary team. Critical to this role is strong communication and strong collaboration with patients, providers, and other members of our team. At least 2 years of experience and exceptional skill is required.

Responsibilities:

• Provide skilled nursing care to patients in various specialties including Medical/Surgical, ICU, ER, Psychiatric, Labor & Delivery, PACU, and Operating Room.
• Conduct comprehensive patient assessments to monitor health status and progress.
• Administer medications and treatments as prescribed and monitor patient responses.
• Develop and implement individualized treatment plans in collaboration with the healthcare team.
• Coordinate case management and provide hospice care to patients with advancing age, frailty, or serious illness.
• Communicate effectively with patients, families, and interdisciplinary team members to ensure quality care.
• Maintain accurate and timely documentation of patient care and treatment plans.

Required Skills & Certifications:

• Active Registered Nurse license in the state of employment.
• Bachelor's degree in Nursing or related field.
• Minimum of two years experience in hospital, home health, or hospice care settings.
• Proficient clinical assessment, medication administration, and treatment planning skills.
• Strong interdisciplinary team collaboration skills.
• Excellent communication and interpersonal abilities.
• Valid driver's license, car insurance, and reliable automobile for field work.

Preferred Skills & Certifications:

• Experience in specialized nursing areas such as ICU, ER, Psychiatric, Labor & Delivery, PACU, or Operating Room.
• Advanced certifications related to nursing specialties or hospice care.
• Additional training in case management or palliative care.

Special Considerations:

• Ability to work effectively in fast-paced and dynamic healthcare environments.
• Flexibility to collaborate with diverse patient populations and healthcare providers.
• May require occasional travel for field work and home health visits.

Scheduling:

• Work schedules may vary depending on the healthcare setting and patient needs.
• Shifts may include day, evening, night, or weekend rotations as required.
• On-call responsibilities may be part of the role depending on the assignment.
